Order Reports were put into place to better help L & T Associate's better understand the packaging and shipments of customers and clients. It is also to let us know when customers and clients have received their packages and to give report if something went wrong or a package wasn't received. This is a good place to put some contact information, so that we can get in touch with you. A Name, Address, Phone, and email on the Form is necessary. If you want to put an address, you can place that in the concerns section of the form.
